L-Arginine Simply Explained
L-arginine is a protein building block that the body produces itself and takes in through food.
1. What Is L-Arginine?
L-arginine is an amino acid, i.e., a building block of proteins. The body can produce it itself. We also take it in through protein-containing foods. It is offered as a dietary supplement and used in medical nutrition.
2. Where Is It Found, and What Is It Made From?
L-arginine is found in proteins of animal and plant foods, for example in meat, fish, dairy products, legumes, nuts and seeds. For preparations, it is produced industrially, mostly via fermentation or from protein hydrolysates. In studies, mainly the free base, the hydrochloride, the alpha-ketoglutarate and the ethyl ester are tested, sometimes as a pure substance, sometimes in mixtures.

Blood Pressure
In a controlled study, the systolic blood pressure of healthy men was lower after an L-arginine drink than after a placebo drink 35. A review found a lower upper blood pressure value in women with preeclampsia taking L-arginine, but the data on this are very uncertain 6. Another review found no difference in blood pressure in at-risk pregnant women 2. Overall, the findings are mixed. The good results mostly come from very small studies. The more reliable reviews that look only at L-arginine show no or only limited benefit.
Cardiovascular Health
In pulmonary hypertension and in a blood disorder called beta-thalassemia, the pressure in the pulmonary artery decreased under L-arginine, but not under placebo (sham treatment) 3414. After a bypass operation, blood flow in the grafts remained more stable under an L-arginine infusion than under placebo 24. Two reviews report more favorable risk factors for the heart and circulation 1 and lower troponin T in bypass patients 8. For heart rate and heart rate variability after exercise, several controlled studies (with a comparison group) found no difference from placebo 192129. The studies were mostly small and short.
Metabolism
L-citrulline in the blood rose under an L-arginine infusion in healthy individuals 27 and under oral L-arginine in pulmonary hypertension 34. In children with sickle cell crisis, intravenous L-arginine improved platelet mitochondrial activity and oxidative stress compared with placebo 13. There was no confirmed difference in opioid requirement and pain, the main objectives of the study 13. Clinical endpoints such as body composition, testosterone or inflammatory markers were inconsistently affected. Several studies were small or short. The evidence is moderate; the significance for hard clinical endpoints remains open.
Pregnancy
A review in at-risk pregnant women found preeclampsia less often with L-arginine than with placebo 2. A review reports, with low certainty data, that L-arginine reduced the risk of preeclampsia in prevention studies and reduced the child's growth disorder in treatment studies 6. A randomized (participants assigned by lot) controlled trial with oral L-arginine in addition to standard therapy found higher clinical and chemical pregnancy rates in women with recurrent implantation failure 15. No differences were found in gestational age or APGAR score 2. A combination preparation with chasteberry, green tea, L-arginine, vitamins and minerals led to pregnancy more often than placebo 3133. This cannot be attributed to L-arginine alone. The transferability to other forms such as L-arginine hydrochloride or ethyl ester is open.
Vascular Function
In pulmonary hypertension, L-arginine lowered the vascular resistance of the lungs 34. After a bypass operation, coronary resistance remained more stable under an infusion than under placebo 24. In women with repeated implantation failure, the resistance of the uterine arteries decreased more under L-arginine than in the control group 15. In heart failure with preserved ejection fraction, a perfusion index after ischemia normalized under L-arginine as under citrulline, without a placebo group 32. In healthy individuals, vasodilation (flow-mediated dilation) remained unchanged under L-arginine 23. In long COVID, a mixture of L-arginine and vitamin C improved flow-mediated dilation compared with placebo; how much of this is due to the L-arginine remains open 17. After a high-fat meal, L-arginine alone or with beetroot powder did not change flow-mediated dilation 25. The forms tested range from the free base to the hydrochloride to L-arginine from protein-rich foods. The studies are predominantly small and short. No results are available for clinical endpoints (study target values) such as heart attack or stroke.
Wound Healing
In inflamed oral mucosa after radiation, L-arginine alleviated severity and pain more than a sham treatment 16. Oral nutritional supplements with L-arginine, omega-3 fatty acids and other substances did not reliably reduce wound infections 20 and pressure ulcers 28.
Other Investigations
In older people with declining mental performance, proportionally more participants scored normally on the cognitive test under L-arginine than under placebo 18. An L-arginine drink prolonged the time to exhaustion 35. One week of L-arginine did not change endurance performance 26. An L-arginine hydrochloride infusion increased urinary nitrate and cGMP excretion in healthy individuals more than placebo 30. In erectile dysfunction, the favorable finding of a review is based on a combination of L-arginine and pycnogenol; this says nothing about L-arginine alone 3. Mainly the free base and the hydrochloride were studied, predominantly orally. Much remains open regarding dose, duration and clinical endpoints for the combination preparations.
4. How Was L-Arginine Taken in the Studies?
The studies predominantly used oral doses of L-arginine as a free base. In addition, L-arginine hydrochloride and L-arginine alpha-ketoglutarate were examined. Individual investigations examined L-arginine from protein-rich foods as well as infusions. The doses and treatment durations are shown in the table.
5. Is L-Arginine Safe? Side Effects in the Studies
Under L-arginine, older participants discontinued individually due to thyroid disorder, nausea with diarrhea or bitter taste, while no one did under placebo 18. A training combination preparation with L-arginine alpha-ketoglutarate, caffeine, beta-alanine and other substances did not change heart rate, blood pressure, ECG and blood values in a clinically significant way 22.
