Sulforaphan (Broccoli) Simply Explained
Sulforaphan is a compound from broccoli and other brassicas. This page explains in simple words what human studies show about it and what remains open.
1. What It Is About
Sulforaphane is a plant substance. It is not present ready-made in the plant but forms only when the cells of broccoli, broccoli sprouts, cauliflower or kale are injured, for example during chewing or chopping. A precursor then encounters an enzyme, that is a protein that initiates a conversion. Without this step the precursor remains ineffective.
Very young sprouts three to five days old supply the most sulforaphane. They contain more of the precursor than mature plants. After ingestion sulforaphane is absorbed rapidly. The highest blood level appears after one to three hours. The body then metabolises the substance and excretes it in the urine.
For research it matters in which form sulforaphane is taken. Fresh sprouts, ready-made extracts, seeds and cooked or raw plant parts deliver different amounts because heat destroys the enzyme that releases the substance. Many studies therefore use preparations with a defined content.
2. What Studies Show
The most frequent question examined so far is whether sulforaphane influences inflammatory markers or safety. Individual studies look at behaviour, cognition or metabolism.
Autism spectrum disorder. In a study with 44 adolescents and young adults, sulforaphane from broccoli sprouts was given daily for 18 weeks; a second group received a placebo, that is a preparation without active substance. Parents and physicians then completed behaviour questionnaires. Ratings improved markedly in the sulforaphane group and changed little in the placebo group. The number of participants was small and they were not followed after the study ended. These indications are not yet confirmed 1.
Cognition in schizophrenia. 172 patients with early-onset schizophrenia took part for 22 weeks. No difference from placebo appeared in the overall test of cognitive performance. In individual domains the sulforaphane group performed better, in spatial memory and logical thinking 3.
Cognition and mood in older adults. In a study with 144 healthy older adults over twelve weeks, participants taking sulforaphane processed information faster and reported less low mood than the placebo group. Blood values indicating inflammation or cell damage remained unchanged 7.
Airways in former smokers. 43 former smokers received sulforaphane for twelve months. In airway tissue samples a marker indicating the rate of cell division declined. The tissue appearance itself did not change 5.
Tolerability. The studies mainly recorded mild gastrointestinal complaints that resolved. No serious adverse effects were reported, including in 16 healthy men over seven days 8.
3. What Remains Unclear
Many investigations measure only blood or tissue values. Whether these changes also improve symptoms often remains open. In COPD patients and in people on dialysis, that is blood purification for kidney failure, the measured values did not change 2 6. In cancer the studies showed no consistent results for disease course 4.
Participant numbers are usually small, study durations short, and the preparations used differ greatly. Whether the observed changes persist beyond one year has not yet been clarified. The study that records both body values and participant well-being over a long period with comparable preparations is therefore still outstanding.
4. What This Means in Everyday Life
Sulforaphane belongs to normal eating: broccoli, broccoli sprouts, cauliflower, kale and Brussels sprouts contain the precursor from which it forms. The fact that a substance occurs in foods says nothing about how it acts as a tested preparation. The human studies on this are small single investigations in selected groups.
A distinction helps with interpretation: changes were observed in people with autism, with schizophrenia, in older adults and in former smokers that warrant further testing. In COPD patients and dialysis patients the measured values did not change. What underlies this difference is still only a hypothesis.
Cabbage vegetables therefore remain a normal food for the time being. Whether sulforaphane will one day be used as a medicinal product depends on the studies still outstanding, above all those that measure well-being and body values together over long periods.

Frequently Asked Questions
How does sulforaphan form in broccoli?
In the plant, only the precursor is initially present. Sulforaphan forms when chewing or chopping brings the precursor into contact with the matching enzyme. Cooking destroys this enzyme, which is why raw and cooked plant parts deliver different amounts.
Is sulforaphan a medicine?
Sulforaphan is a plant compound, not a medicine. In the studies, participants receive it from sprouts, seeds or an extract with a defined content, which cannot be compared one-to-one with a broccoli dish. Whether it will one day become an approved medicine remains open.
Are there side effects?
In the studies, participants mainly reported mild gastrointestinal complaints that resolved again. No serious side effects were reported. The observation periods were, however, short.
