Piperine Simply Explained

Piperine is a substance from black pepper. This page explains in simple words what human studies show about it — and why curcumin is almost always involved.

1. What It Is About

Piperine is the best-known substance in black pepper. More precisely, it is the principal alkaloid of black pepper — alkaloids are bitter plant substances that contain nitrogen. Piperine is not a nutrient like a vitamin. It belongs to the secondary plant substances, that is, substances a plant forms for itself.

In studies piperine appears in two roles. Once as a candidate for an effect in its own right. And again as a companion substance that alters the uptake of other substances. This dual role runs through almost all the work.

One point stands out: the most frequently cited human studies almost always test piperine together with curcuminoids, the colouring agents of turmeric. What was examined, therefore, was the mixture, not piperine alone. Anyone reading a headline about piperine is usually reading a headline about a combination.

2. What Studies Show

The combination of curcuminoids and piperine is the best studied. A large review has pooled and jointly evaluated many controlled trials of this combination 1. Such reviews carry the greatest weight because they bundle numerous individual investigations. The topics were inflammation and oxidative stress — that is, the formation of aggressive substances in the body and the resulting defence reactions.

In people with type 2 diabetes, 118 participants took curcuminoids for eight weeks 2. Blood values that indicate the burden of aggressive substances were measured. In a further study with 100 participants with type 2 diabetes, curcuminoids were given together with piperine 3. Values for blood glucose, liver and inflammation were reported.

In a study with 70 participants with fatty liver, liver values improved under curcuminoids with piperine 4. In people on dialysis, one study compared turmeric with piperine against turmeric alone 5. The combination with piperine performed better on the inflammation values than turmeric alone.

Two controlled human studies address tolerability. In one of them, 96 participants with severe overweight received a plant extract together with piperine for over 90 days. No adverse events were reported and blood values remained within the normal range.

All these results share the same key point: piperine was never the only substance. It was added so that the other substance would be absorbed better. Any improvement therefore cannot be attributed to piperine alone. The studies themselves do not claim this either.

3. What Remains Unclear

The greatest weakness of the research is the combination. When piperine is given together with curcuminoids or in mixtures of several substances, it is no longer possible to say which substance produces which effect. Only a few controlled studies test piperine as the sole test substance.

Many results are also blood values and questionnaires. How a disease actually progresses is recorded less often.

A second point remains open: piperine is said to inhibit a certain protein that breaks down medicines in the body. This finding comes from experiments with human cell preparations. Whether piperine acts similarly in humans is an open question. Likewise, no amount at which side-effects occur can be derived from the studies conducted so far.

4. What This Means in Everyday Life

Anyone reading about piperine should pay attention to the title of the study. If it mentions piperine together with curcuminoids, the results belong to the combination. That is the core of the matter: piperine is almost always tested as a helper, rarely as the main actor.

For everyday life this means: statements such as “piperine works against inflammation” are usually based on studies in which another substance was also present. What piperine can achieve on its own has therefore not been clarified.

What needs to be clarified next: a controlled study in which piperine is the only substance. The extent to which the uptake of a companion substance changes should be measured. It also remains open whether piperine influences the breakdown of medicines taken at the same time.

Frequently Asked Questions

Is Piperine the Same as Black Pepper?

No. Piperine is the best-known substance in black pepper, but pepper contains many other things. A study on piperine therefore does not automatically say something about pepper.

Why Is Piperine So Often Given Together with Curcumin?

Piperine is considered a helper that can improve the absorption of other substances. That is why it is in many studies alongside curcuminoids. What is investigated is then the mixture, not piperine alone.

Can Piperine Change the Effect of Medicines?

This is an open question. In experiments with human cell samples, piperine inhibited a protein that breaks down medicines. Whether this happens in the same way in humans has not yet been clarified.
