Mindfulness and Meditation (MBSR) Explained Simply

MBSR is an eight-week group programme combining meditation, body exercises and yoga. This page summarises in simple terms what research shows about it — and what remains open.

1. What It Is About

MBSR stands for Mindfulness-Based Stress Reduction. It is a group programme developed in the 1970s by Jon Kabat-Zinn. It combines meditation, body awareness exercises — known as the Body Scan — and elements of yoga. Its aim is to foster a more conscious perception of the present moment. Originally designed for people with chronic pain, the programme is now used in hospitals, rehabilitation centres and workplace health programmes. A typical course lasts eight weeks, with one session per week and daily home practice.

In addition to the classic MBSR, there are variations such as Mindfulness-Based Cognitive Therapy (MBCT) or shorter online formats. The programmes differ in duration, setting and target group. Measurement methods also vary: some studies use questionnaires on stress or quality of life, while others assess physical parameters such as cortisol levels or cognitive tests. This diversity makes it difficult to compare results directly.

2. What Research Shows

MBSR is best studied in relation to anxiety, stress and pain, with several controlled trials available.

In a study of 276 adults with an anxiety disorder, the eight-week programme performed similarly to the medication escitalopram after eight weeks 2. Participants were aware of which treatment they received.

In a study of 250 adults with chronic pain who were misusing pain medication, an eight-week mindfulness-based programme with weekly two-hour group sessions was tested. Nine months later, this group showed greater improvements in pain medication use and pain burden than a comparison group receiving supportive group therapy 1.

Among 39 carers of people with dementia, a six-week mindfulness training programme led to improvements in compassion fatigue and burnout; these effects remained stable three months later 7. In a study of 60 breast cancer patients, participants reported fewer subjective memory problems two weeks after the course, while objective memory tests showed no change 9. A review of 25 studies involving lung cancer patients also found reduced anxiety and depression 4.

There is some evidence for blood pressure, though it is not yet conclusive: in 42 people with hypertension, lower blood pressure readings over the course of a day were measured after eight weeks of MBSR compared to a control group 5. A review of 18 studies found MBSR to be superior to usual care in reducing pain symptoms and disease burden in fibromyalgia; results for back pain and headaches were less consistent 6.

3. What Remains Unclear

The studies are difficult to compare because the programmes vary in length and are delivered either online or in person. Measurement methods also differ: some use questionnaires, others physical parameters. Many studies lack an active control group, and participants are usually aware they are taking part in the programme — making it hard to separate expectation effects.

Two pooled studies involving 218 participants found no changes in brain structure after eight weeks of MBSR 8. In 585 older adults with subjective memory complaints, a study that combined mindfulness training and exercise found no differences in objective memory tests after 18 months, though subjective relief was reported 3. How long the observed improvements last and whether they also apply to populations rarely included in studies remains to be determined.

4. What This Means in Everyday Life

The observed improvements largely relate to self-reported outcomes: anxiety, stress, exhaustion, sleep and quality of life. Where objective measures were used, results were more mixed — there was some evidence for blood pressure, but not for memory tests or brain structure.

MBSR is a group programme lasting eight weeks, not a single exercise. To interpret the research, it is important to consider who was studied and what was measured. The next step would be to clarify how mindfulness programmes affect people over several years and how they compare directly with other approaches.

Frequently Asked Questions

Do I need prior meditation experience?

No. In the studies, participants without meditation experience took part, for example 218 people in two combined studies 8.

How long does an MBSR course take?

Typically eight weeks with one group session per week and daily home practice. Some studies tested shorter programmes of six weeks or purely online formats.

What does it mean that MBSR was ‘not inferior’ to a medication?

In a study with 276 adults with an anxiety disorder, improvements after eight weeks of MBSR were compared with those of the medication escitalopram 2. Both groups showed similar results. Participants knew which treatment they were receiving.
