Alpha-Lipoic Acid Explained Simply

Alpha-lipoic acid is a substance produced by the body that helps in cells with energy production and is offered as a dietary supplement.

Last updated:

1. What Is Alpha-Lipoic Acid?

Alpha-lipoic acid is a sulfur-containing substance. The body itself produces small amounts of it. In cells, it helps convert sugar into energy. It is also offered as a dietary supplement. In some countries, approved medicines are additionally available.

2. Where Is It Found, and What Is It Made From?

Alpha-lipoic acid occurs in small amounts in foods. These include spinach, broccoli, potatoes and yeast. It is also contained in liver and kidney. For dietary supplements and study preparations, it is chemically produced. It usually occurs as a mixture of both enantiomers; only the R form occurs in the body.

Oxidative Stress (Laboratory Markers)

In several review articles, inflammation markers in the blood decreased 93229. Antioxidative capacity also improved in some studies 49526063. Other studies found no difference here 182548505761. These results are laboratory values and not clinical effects. The studies were often small and inconsistent.

Glucose and Fat Metabolism

Several reviews show that alpha-lipoic acid can lower fasting blood glucose 213137 and long-term blood glucose 2131. Two others found no difference in long-term blood glucose 1423. Insulin resistance also improved in several studies 21375664. For cholesterol, there were partly improvements 152433, partly no difference 121416. The studies were heterogeneous and mostly short.

Nerve Damage, Especially in Diabetes

In diabetic nerve damage, alpha-lipoic acid improved symptoms in several reviews 52023262839. Pain also decreased in some studies 34412328. However, two reviews with placebo comparison (sham treatment) found no confirmed difference 1127. Combinations with other agents cannot be attributed to alpha-lipoic acid alone 236710. For other nerve damage, the benefit was unclear.

Weight and Body Composition

Oral alpha-lipoic acid slightly lowered body weight in several reviews and studies 21354043. A review also reports a decrease in fat mass 21. In chronic kidney disease and polycystic ovary syndrome, however, BMI remained unaffected 2425. The findings apply to oral intake.

Liver

In fatty liver, a review found no advantage for liver enzymes 16. A randomized (participants assigned by lot) study also showed no difference in liver enzymes and liver fat 55. After liver transplantation, circulatory problems upon restoring blood flow (post-reperfusion syndrome) occurred less often with alpha-lipoic acid than with placebo 58. The studies are small and short. Overall, there was predominantly no advantage.

Eye and Retina

For the eye, only one randomized controlled study (with a comparison group) is available. After acute optic neuritis, the retinal nerve fiber layer decreased with alpha-lipoic acid as with placebo 59. The study was small and short. Reliable statements about retinal function cannot be derived from it. A case series (description of several individual cases) reports improvements, but only for a mixture with other substances 65.

Other Investigations

In this area, very different endpoints (study target value) were examined, predominantly with oral intake. Several studies found less pain, for example in migraine, burning mouth syndrome and hemorrhoids 283638424446. In men with fertility disorders, sperm motility improved 18196263. Vascular function improved in several studies 174764, blood pressure decreased in a review and two randomized studies 135154. Mood and depression scores improved in two randomized studies 4852.

4. How Was Alpha-Lipoic Acid Taken in the Studies?

In the studies, alpha-lipoic acid was predominantly given orally as a tablet, capsule or dietary supplement. The summaries mostly do not state which form was used. Individual investigations administered alpha-lipoic acid intravenously, especially in diabetic nerve damage. The two mirror-image forms were not examined separately in the studies with a comparison group.

5. Is Alpha-Lipoic Acid Safe? Side Effects in the Studies

The studies predominantly report no serious adverse events and no difference in treatment discontinuations due to side effects 111. Side effects occurred at higher doses; at lower doses, administration was considered safe 26. Case reports describe an insulin autoimmune syndrome after intake, which subsided after discontinuation 30. Findings on laboratory values such as oxidative stress are not clinical effects.
