Lifestyle Programme Simply Explained
A lifestyle programme combines nutrition, movement and behaviour change into a joint plan. This page explains in simple words what studies show about it and what remains open.
1. What It Is About
A lifestyle programme is not a single measure. It is a plan that addresses several areas of everyday life at the same time: nutrition, physical activity, weight management and, depending on the target group, also sleep, stress management or smoking cessation. In addition, there are usually elements that support behaviour, for example self-monitoring, goal setting and feedback from professionals or via digital applications. Some programmes run in personal sessions, others in groups, by telephone or via the internet. Some are based on well-known curricula, such as the Diabetes Prevention Program, others are developed for specific groups, for example for pregnant women, children, older people or people with chronic conditions. The duration ranges from a few weeks to several years. Because the term encompasses so many different things, studies can only be compared to a limited extent.
2. What Studies Show
The field of metabolism is the best studied. A summary of 14 controlled studies with adults who were overweight or severely overweight found that participants on a lifestyle programme lost on average about two and a half kilograms more than the comparison group 1. The programmes lasted at most six months; some were delivered remotely, others in person. A large Cochrane review on polycystic ovary syndrome, a common hormonal disorder in women of childbearing age, names weight management through nutrition, physical activity and behavioural measures as the first treatment strategy 2. In a controlled study with 83 people with rheumatoid arthritis, an inflammatory joint disease, the disease score improved more after 16 weeks than under usual care; the programme consisted of a whole-food plant-based diet, physical activity and stress management 4. Weight, fat mass and waist circumference also decreased more. Another study with 174 cancer patients and their relatives showed more steps after eight and after 20 weeks among participants in the programme 6. In a study with 228 older, severely overweight adults, trained lay people from the respective retirement centre led the group sessions 3.
3. What Remains Unclear
The term lifestyle programme stands for very different offerings. The number and type of components, intensity, duration and form of support differ greatly. Therefore, a result from one programme cannot be readily transferred to another. A finding on an internet-supported programme for young adults says little about a group meeting for older people. It is also open which component makes the difference: whether coaching, text messages or an app cannot be decided with the data available so far. In the TODAY study with 234 adolescents with type 2 diabetes, participants attended only about 60 per cent of the planned appointments, and self-monitoring of nutrition and physical activity was rare 5. How programmes can be designed more effectively for people with lower formal education is also still unclear.
4. What This Means in Everyday Life
The research indicates that programmes lasting several months with nutrition and physical activity components reduce weight compared with a control group. This is a recurring finding from several controlled studies. It is less clear which individual components contribute to this and which format suits whom best. Anyone interested in such a programme will find offerings in very different forms, from personal sessions to internet-supported variants. The question is less whether lifestyle programmes work, but which components make a difference for which group under which conditions. This is precisely the open question that further studies should clarify.

Frequently Asked Questions
What does a lifestyle programme include?
Typical elements are nutrition, movement and weight management. Depending on the target group, sleep, stress management or smoking cessation may be added. Most programmes also include elements that support behaviour, such as goal setting and feedback.
How long does such a programme last?
This varies greatly. Some programmes run for only a few weeks, others for several months or years. In a summary of 14 studies, all lasted at most six months 1.
Are lifestyle programmes scientifically studied?
Yes, there are many human studies, including 211 controlled trials. Metabolism and fatigue are the best studied. However, the programmes differ greatly, which makes comparison difficult.
What is still unclear?
It remains open which component of a programme makes the difference. Whether coaching, text messages or an app carries the effect cannot be decided with the data available so far.
